Referral Form
If you wish to refer a patient to Hospis Malaysia, kindly ensure that the referral is made by a specialist or a family doctor. As this service works best as a complement to medical care, a proper referral can provide us with relevant medical information about the patient. This helps our palliative care team to arrange a patient management plan based on the patient’s health condition.
Once a referral is accepted, a palliative care nurse will be assigned to manage the patient’s day-to-day care. The patient and family will be visited, assessed, and provided with explanation of their respective roles. Other healthcare professionals such as our palliative doctors, clinical pharmacist, occupational therapist, and designated volunteers may also be involved in patient care. The team will propose a patient’s management plan to address the areas of concern and organise frequent case reviews during ongoing care.
Number of visits given from the nurse assigned vary based on the needs of each patient and the family. For any emergencies, Hospis Malaysia also provides a 24-hour on-call service for registered patients. Out-of-hours calls will be handed to an on-call palliative care nurse. Upon death of a patient, a nurse will arrange bereavement visits. Once a case is closed, a discharge summary will be sent to the respective referring doctor.
